To qualify for our program, your child must:

• Be age 0 to 21
• Live in Kern County

Have at least one of the following:

• Born early (37 weeks or less)
• Born small (weighed 5.5 pounds or less)
• Spent time in a NICU (Neonatal Intensive Care Unit)
• Has a health condition that needs long-term care
• Does not have health insurance
• Needs extra support because of medical, developmental, family, or financial challenges

Our team helps families:

• Find doctors and specialists who understand your child’s needs
• Apply for health insurance or programs that can help
• Connect with services and resources like therapy or transportation
• Create a medical “home” — a place that manages all your child’s care
• Navigate the health system so it’s easier and less stressful
